Copyright Request Form

The American Heart Association/American Stroke Association currently does not allow 3rd parties to use/reproduce AHA/ASA Copyrighted material in phone and/or notebook applications.

This automated permissions process will take you through the following steps:

1. Specific material to be reproduced
2. Contact information
3. Details of how material will be used
4. Preview request
5. Submit request
6. Option to make another request


If the requested material is not one of the following four options then please proceed to the next section.

Journal Article (Non-statement or Guideline Document)

Link to American Heart Association or American Stroke Associaton web site

Logo or other service marks (examples Red Dress mark, Start! mark, etc.)

Phone/Notebook Application


ATTENTION:
As of August 25, 2014, all copyright permission requestors will be directed to an AHA secure login screen before you will be able to fill out a request form; you will only be ask to set up your account one-time. The Copyright Request process is moving to SharePoint so the login will be required. The request form will be self-explanatory, as before, and will have a few more options. The major change is in the order of the request; on the new form your contact information will be requested last, instead of first.
